Why this matters
Performance-enhancing substances (PES) — such as peptides, testosterone therapy, stem cells, and other modern interventions — are no longer confined to elite athletes. Today, they are widely used for wellness, functional health, and longevity.
People are turning to PES to enhance energy, recovery, sleep, and resilience; to regulate hormones and prevent illness; and to explore anti-ageing strategies. This reflects a significant shift in the way society views health and performance.
However, this rapid growth has also created challenges. Information is often inconsistent, shaped by social media hype, celebrity endorsements, and commercial marketing. At the same time, traditional medical and policy responses tend to focus on condemnation and stigma rather than support. This leaves many individuals without access to reliable guidance, creating significant risks to their health and well-being.
A new perspective
- Current harm reduction models are outdated. They were designed mainly to deter drug use in competitive sports, but they fail to address the wider and growing reality: PES are now part of mainstream health, wellness, and longevity practices.
- The key challenges include:
- Stigma and condemnation – users are often judged, which discourages them from seeking medical help.
- Conflicting advice – research results, regulations, and public messages often contradict each other.
- Ease of access – PES are widely available online and through private clinics, but oversight is inconsistent.
- Influence of celebrities and social media – PES use is often normalised in public spaces, but without balanced education or context.
These issues create a gap between real-world use and credible, science-based harm reduction support.

SAFE PULSE – building a global framework
SAFE PULSE (Safe Performance, Uplift, Longevity, Safety, Education) is an international initiative co-founded by Imran Khan, Dr Luke Cox, and Dr Timothy Piatkowski.
Its mission is to provide a modern, research-driven, and non-judgmental approach to harm reduction and human enhancement.
The ethos of SAFE PULSE is straightforward:

- Performance uplift – supporting responsible optimisation.
- Longevity – prioritising healthspan over short-term gains.
- Education – cutting through misinformation with accessible, evidence-based knowledge.
SAFE PULSE is designed to be a global leader, with a presence in the UK, Europe, Australia, and South Asia.

Yet despite these benefits, social stigma and outdated attitudes remain. The reality is that PES are becoming part of mainstream health culture, but the frameworks for safety and education have not kept pace with this development. This creates the need for a new paradigm — one that balances opportunity with responsibility.
